Why Falls Happen and How to Prevent Them as We Age

Falls are one of the most common, and most preventable, events I encounter in my work with older adults. A single fall can change the entire trajectory of a person’s independence, sometimes leading to hospitalization, loss of confidence, or a permanent move to a higher level of care. Across my years in skilled nursing, assisted living, and home health, I’ve found that most falls result from a combination of factors rather than a single cause.

Muscle weakness, poor balance, and slowed reaction time are major contributors, especially after a period of illness, surgery, or inactivity. Vision changes, medication side effects such as dizziness or low blood pressure, and environmental hazards like clutter, loose rugs, or poor lighting also play a significant role. In my home health visits, I often find that the layout of a person’s own house, one they have lived in for decades, has quietly become a hazard as their mobility and balance have changed over time.

Prevention starts with an honest assessment of strength, balance, and gait, which is exactly what occupational and physical therapists are trained to evaluate. Simple interventions can make a real difference, including removing tripping hazards, improving lighting, installing grab bars, wearing supportive shoes, and doing targeted balance and strengthening exercises.
